AI SEO for Healthcare: Get Your Practice Cited

AI SEO for healthcare is not a new trick layered on top of SEO. ChatGPT, Gemini, Perplexity, Copilot and AI Overviews all retrieve from search indexes first, then decide which sources they trust enough to quote, and their agents now go further and act for the patient. I work on all three: being retrievable, being the source an engine is confident naming, and agentic optimisation so an AI agent can complete a booking on your site.

How I measure AI visibility

AI answers change from one day to the next, so a single screenshot proves nothing. I track a fixed set of 20 to 50 questions your patients or referrers actually ask, written for your specialty and your cities, and run them every month in ChatGPT, Perplexity, Gemini and Google’s AI Overviews and AI Mode.

Each run is logged the same way: memory and personalisation off, the country named in the question, and a record of who is named, who is linked as a source, and why the engine says it chose them. That last part is the useful one. It tells us which page, review or mention to build next.

You see the same log I use for my own site, measured in public. What I report is the share of questions where you are named or cited, next to your competitors, and the enquiries that follow. Never a single lucky answer.

How AI engines choose which clinic to name

Every major engine starts with retrieval. AI Overviews and Gemini draw on Google’s index. ChatGPT search and Copilot lean on Bing’s. Perplexity runs its own crawler alongside others. If your pages can’t be fetched, rendered and indexed by the right crawler, nothing else on this page matters.

After retrieval comes confidence. An engine prefers sources whose facts agree everywhere it looks: the same practice name, clinicians, specialties and locations on your site, your Google profile, directories and the third-party pages it already trusts. Then it extracts a passage. Pages written as one long narrative lose to pages where each question has a clear, self-contained answer.

Agentic optimisation: when AI acts for the patient

AI no longer only answers. Agents such as ChatGPT’s agent, Gemini and Google’s AI Mode can open your site, compare clinics, check insurance and start a booking on a patient’s behalf. An agent does not guess. If it cannot read your hours, fees or availability, or cannot get through your booking form, it moves on to a clinic whose site it can use.

Agentic optimisation is making every fact your patients care about machine-readable and every step of the booking path completable by software as well as by people. It is the next layer on top of AI SEO, and most healthcare sites are not ready for it.

Where AI search goes wrong for healthcare

Blocking the wrong bot

Many sites block every AI user agent to stop training and remove themselves from AI search answers at the same time. The crawlers for search and for training are different, and the choice should be made per engine.

Facts that disagree

A clinician listed at two practices, old addresses in directories, or specialties that differ between site and profile lower an engine’s confidence. It will quote a directory instead of you.

Thin pages at scale

Hundreds of near-identical condition pages written by AI are what Google’s scaled content abuse policy targets. They also give engines nothing new to cite.

The grey side, and why it matters to you

I know how AI answers are being gamed right now: hidden prompt text in pages, self-published “best clinic” lists on sister domains, paid placements dressed as editorial, and bulk AI content parked on authoritative sites, which Google treats as site reputation abuse. I don’t use any of it. Knowing how it works is how I keep your visibility clean when engines start discounting it, and they do.

What is agentic optimisation?

Preparing your site for AI agents that act for patients: comparing clinics, checking insurance and starting bookings. It means facts an agent can read and a booking path it can complete, tested with the agents patients actually use.

Can you guarantee ChatGPT will recommend my clinic?

No one honestly can. Answers vary by prompt, location and engine. What I can control is whether you are retrievable, consistent and quotable, and I measure the share of answers you appear in every month.

Is AEO or GEO different from SEO?

It is SEO with a different finish line. The foundations (crawlable pages, clear entities, authority) are the same. The difference is writing and structuring for passage extraction and third-party confirmation, not only for a click.

Should I block AI crawlers?

Block training crawlers if you prefer, but keep the search crawlers open, or you disappear from AI search answers. I set this per engine.

Does llms.txt help?

It is harmless to add, but no major engine has committed to reading it, so it is not where results come from.
