LegitScript Certification SEO: What It Changes for Rehab

LegitScript certification SEO is a question I hear from almost every addiction treatment centre. Certification is required to run Google Ads for addiction services in the US and Canada, but its real value for organic search is what the process forces you to fix: transparent locations, verified licensing, no patient brokering. Those are the same signals that separate a real facility from lead-gen spam.

What Google actually requires

Google’s healthcare and medicines advertising policy requires certification to advertise recovery-oriented drug and alcohol addiction services in several countries, including the United States, Canada and Australia. In the US and Canada, that certification comes through LegitScript’s Addiction Treatment Certification program. Google also notes that not every provider qualifies, and a provider that cannot get certified cannot advertise these services on Google Ads.

Certification is an advertising requirement, not an organic ranking factor. Nothing in Google’s documentation says certified sites rank higher. So why does it matter for SEO? Because of what it takes to pass.

In other words, a site that is ready for certification is a site that looks like what it is: a licensed facility with real clinicians at real addresses. That is exactly what Google’s quality systems try to reward in this sector.

Why rehab search is different: the spam problem

Addiction treatment search has been one of the most abused areas of local search. In 2020, The Register reported that researchers had found hundreds of fake Google Business Profile listings for substance use treatment, many routing callers to call centres or to affiliated facilities far away. In one search for alcohol detox, most of the top results were not real local businesses. Google said at the time that it monitors sensitive business types such as healthcare facilities closely.

For a legitimate centre, this means two things. First, Google is sceptical of the category, so your listing and site need to prove you are real. Second, your competitors may not be real facilities at all.

How I build organic visibility for a certified (or certifying) centre

  1. One page per facility and per level of care. Detox, residential, partial hospitalisation, intensive outpatient and outpatient are different searches with different intent. Each gets its own page, stating where it is offered and who it is for. This gives Google one clear page to rank for each intent instead of one vague “programs” page.
  2. Licences and accreditations visible and linked. State licence numbers, and accreditation from bodies such as The Joint Commission or CARF where the centre holds it, shown on each facility page and linked to the issuing body’s public record where one exists.
  3. Clinicians as real people. Profiles for medical and clinical leads with credentials, and clinical review dates on treatment content.
  4. Schema that matches reality. A MedicalClinic or MedicalOrganization graph per facility, with address, hours and the services offered there. No schema for services a location does not provide.
  5. Google Business Profiles that pass scrutiny. Real signage, a staffed address, a local phone number and categories that match the licence. Details in my guide to recovering a suspended Google Business Profile.
  6. Content written for families as well as patients. Insurance, what the first day looks like, visiting and aftercare. No outcome guarantees and no success rates you cannot substantiate.

Patient brokering and EKRA: where marketing becomes a crime

The Eliminating Kickbacks in Recovery Act, 18 U.S.C. § 220, makes it a federal crime to knowingly and wilfully pay or receive remuneration for referrals to recovery homes, clinical treatment facilities or laboratories. Penalties reach fines of up to $200,000 and up to 10 years in prison per occurrence. Several states have their own patient brokering laws as well.

This matters for SEO because some “marketing” in this sector is referral trading in disguise: pay-per-admission lead deals, call centres that sell callers to the highest bidder, and directories that route calls. I don’t work on any referral-based lead scheme, and when I audit a centre’s marketing I flag any arrangement where payment depends on admissions so their counsel can review it.

Spotting and reporting lead-gen spam against you

Signs a competing listing is not a real facility: a residential address, a generic name built from keywords (“Best Alcohol Detox Near City”), a phone number that reaches a call centre, and no licence on the linked site. Google documents how to report a business on Google Maps, and also runs a Business Redressal Complaint Form for listings that break its guidelines. I document the evidence before reporting, because a well-evidenced report is far more likely to be acted on.

Privacy: HIPAA and 42 CFR Part 2

Substance use disorder treatment records carry extra confidentiality protection under 42 CFR Part 2, on top of HIPAA. For marketing, the practical rule is the same one I apply to all healthcare sites: no ad pixels on admissions, insurance verification or intake pages, and no condition or substance names sent to analytics. Questions I get asked

Does LegitScript certification improve organic rankings?

Not directly. Google does not list it as a ranking factor. The work needed to get certified improves the trust and entity signals that do matter.

Can I run Google Ads for my rehab without LegitScript?

In the US and Canada, not for recovery-oriented addiction services. Google’s policy requires certification through LegitScript.

Are pay-per-admission lead deals legal?

Paying for referrals to treatment facilities can breach EKRA and state patient brokering laws. Ask your counsel before signing any deal where payment depends on admissions.
