Rehab Center SEO: A Sample 12-Month Engagement Plan
How I would rebuild organic and AI search for a mid-sized addiction treatment provider with detox, residential and outpatient programmes, where paid search is restricted and every admission is worth thousands. This is a sample engagement plan with an illustrative starting point, written in the same detail I use with clients.
This is a sample engagement plan, not a client result. The provider, its starting numbers and the outcomes are an illustrative model of a two-site addiction treatment provider in the United States. It shows how I plan, sequence and measure an engagement. Client results are shared on a call, under NDA.
Category: Addiction treatment (detox, residential, PHP, IOP) · Market: United States, admits from three neighbouring states · Sites: 2 · Engagement: 12 months · Core work: technical SEO, compliance-led content, levels-of-care architecture, local SEO, entity and accreditation signals, AI search visibility, measurement that respects 42 CFR Part 2 and HIPAA.
The situation
The provider had good clinical standing and weak search presence. Its sample starting point:
| Metric | Start (illustrative) |
|---|---|
| Organic sessions | 6,400 / month |
| Non-branded organic clicks | 1,150 / month |
| Commercial keywords in top 10 | 14 |
| Qualified admissions enquiries from organic | 22 / month |
| Named in AI answers (40-prompt set, 5 engines) | 3 of 200 answers |
| Share of calls via third-party “helplines” | Unknown, suspected high |
Three problems sat underneath. First, the site described the facility rather than answering what families search. Second, paid search was limited, and competitors without the provider’s credentials were capturing branded and local searches through aggregator sites. Third, nobody could say which page produced an admission, because tracking had been stripped back for privacy without a compliant replacement.
Why rehab SEO is different
- Restricted ads. Google requires addiction treatment advertisers to hold LegitScript certification, and several states regulate patient brokering and marketing. Organic visibility carries more of the load than in most sectors. I cover the detail in LegitScript and rehab SEO.
- Two audiences. The person who needs care and the family member searching for them use different language and different devices, often late at night.
- Payer filtering. “Rehab that takes Cigna” or “Medicaid detox near me” is where many decisions are made.
- Trust is checked. Families and AI engines both look for state licensing, accreditation (Joint Commission or CARF), medical leadership and SAMHSA treatment locator listing.
Mapping demand by decision stage
| Layer | Example searches and prompts | Page type |
|---|---|---|
| Crisis and safety | alcohol withdrawal symptoms timeline, is it safe to detox at home | Medically reviewed guides that cite SAMHSA and NIAAA, with a clear route to call |
| Substance | fentanyl detox, benzodiazepine taper, meth addiction treatment | Substance pages with withdrawal risks and the provider’s protocol |
| Level of care | medical detox, residential rehab, PHP vs IOP | Level-of-care pages linked to ASAM criteria |
| Population | rehab for professionals, young adult rehab, dual diagnosis treatment | Programme pages backed by real tracks |
| Payer and cost | rehab that takes Aetna, how much does residential rehab cost | Insurance pages per major payer, honest cost explainer |
| Provider choice and local | best detox in Phoenix, accredited rehab near me | Location pages, accreditation and team pages, Google Business Profile |
| Family | how to get someone into rehab, what to pack for rehab, visiting rules | Family hub |
The audit: what mattered
A full audit finds well over a hundred items. The ones that move admissions in a sample like this:
- Cannibalisation: four blog posts and two service pages all targeting “alcohol detox”. Consolidated into one level-of-care page and one medically reviewed withdrawal guide.
- Thin location pages that swapped the city name. Rebuilt with real facility detail, directions, admission process and staff.
- Credentials in images: licence numbers and accreditation badges were logos only. Moved into text with links to verification sources.
- Insurance information buried in a single verification form. Split into payer pages that state what is typically covered and how verification works.
- AI crawler blocks from a security plugin. Fixed at firewall level.
- Brand confusion: aggregator sites using the provider’s name to route calls elsewhere. Documented, reported, and countered with stronger branded pages and Google Business Profile.
Architecture: levels of care at the centre
The site was reorganised around one path: substance or situation → level of care → programme → location → admission. Example: fentanyl dependence → medical detox → residential programme with MAT → Phoenix site → insurance verification and call. Each step links forward and back, so a family member reading a withdrawal guide can reach the right programme in two clicks.
One URL per intent. Where Google treats “alcohol detox”, “alcohol detox center” and “medical alcohol detox” as the same intent, they share one page.
Compliance built into content
- Every clinical page names its medical reviewer and review date, and cites SAMHSA, NIDA, NIAAA or ASAM.
- No outcome guarantees or “cure” language; success rates only with a stated method.
- Tracking redesigned so no health information reaches ad or analytics vendors without a BAA. My approach is in HIPAA compliant Google Analytics.
- Calls handled by the provider’s own admissions team with clear disclosure, never routed through brokers.
The AI search layer
Families increasingly ask assistants “Which rehabs near Phoenix take Blue Cross and offer medication-assisted treatment?” Engines expand that into searches about accreditation, payers, MAT and location, and cross-check the answer against directories. The plan adds:
- A 40-prompt set across crisis, level of care, payer and provider-choice questions, run monthly in five engines.
- Answer-first sections on every programme and payer page.
- Entity reconciliation across the state licence record, SAMHSA FindTreatment.gov, Joint Commission or CARF directory, Google Business Profile, Psychology Today and insurer directories.
- Third-party corroboration: local news on community programmes, alumni stories with consent, clinician expert quotes in health media.
The mechanics are explained in how AI search chooses healthcare providers.
Month by month (illustrative)
Months 1 to 3: foundations. Audit, crawler access, consolidation, tracking rebuild, credentials into text, Google Business Profile clean-up, first level-of-care and payer pages. Organic enquiries 22 to 29.
Months 4 to 6: depth. Substance pages, family hub, programme pages, medically reviewed guides. First AI answers naming the provider for payer prompts. Enquiries to 38.
Months 7 to 9: authority. Digital PR around a local data angle, clinician media quotes, directory accuracy, review flow. Enquiries to 49.
Months 10 to 12: concentration. Push positions 8 to 20 into the top five, refresh pages with traction, strengthen location pages. Enquiries to 61.
12-month target model
| Metric | Month 0 | Month 12 |
|---|---|---|
| Organic sessions | 6,400 | 15,800 |
| Non-branded clicks | 1,150 | 4,300 |
| Commercial keywords top 10 | 14 | 72 |
| Qualified admissions enquiries (organic + AI) | 22 | 61 |
| Named in AI answers (of 200) | 3 | 41 |
Numbers are a planning model, not a promise. Real results depend on clinical capacity, payer mix, competition and how quickly changes ship.
What I would not do
- Buy links from “recovery resource” networks or pay for placement on lead-generation directories.
- Publish hundreds of city pages for places the provider does not serve.
- Use fake reviews, alumni testimonials without consent, or misleading helpline numbers.
- Track calls in ways that send health data to vendors without a BAA.

Questions I get asked
How long does SEO take for a rehab center?
Technical and consolidation fixes can show movement in two to three months. Competitive commercial terms such as detox or rehab in a metro area usually take six to twelve months of content and authority work.
Do rehab centers need LegitScript for SEO?
Not for organic rankings. LegitScript certification is required to run Google Ads for addiction treatment, and it is a trust signal families and some platforms check.
Can a rehab center appear in ChatGPT answers?
Yes, for provider-choice and payer prompts, when licensing, accreditation, programmes and insurance are stated clearly on the site and confirmed by directories and third-party sources.
Is this plan based on a real client?
No. It is an illustrative model. Real client results are shared on a call under NDA.
